Reorder Materials

Our referral materials are designed to make the referral process as practical and convenient. Referrals can be submitted online, faxed or printed and brought with the patient. Material that can be ordered consist of the following:

Referral Pads:  Please indicate how many you need and they will be delivered to you. You can also send the referrals online with our electronic forms.

Padded Envelopes for Implant Parts:  This stamped padded envelope can be used to return any implant components that you no longer need.

Pano Envelopes (5.5” x 12”):  The stamped pano envelopes allows you to send us any panoramic radiographs of your patients in order for us to be prepared for their upcoming visit.  If you have digital radiographs they can be emailed to us at patientcare@carolinaoral.com.

Carolina OMSC Brochures: Our customized practice brochure introduces Dr. Adams and our practice to your patients for their review prior to their consultation visit.

Implant Consultation Cards:  Know a patient looking to replace a tooth? These cards can be used as post cards to your patient in mind offering them a complimentary consultation with Dr. Adams.
